Provide compassionate, in-home nursing care in accordance with individualized care plans.
Conduct assessments, administer medications, and perform wound care and treatments.
Educate patients and families on managing health conditions and maintaining independence.
Supervise LPNs and Home Health Aides, ensuring quality and continuity of care.
Collaborate closely with an interdisciplinary team to deliver coordinated, high-quality care.
Document patient progress and update care plans to reflect changing needs.
Active Registered Nurse (RN) license in the State of Arizona.
At least one year of nursing experience (home health, hospice, or acute care preferred).
Current CPR certification and valid driver’s license with reliable transportation.
Compassion, excellent communication skills, and a strong sense of accountability.
