Job Description

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans. **SUMMARY** The Case Manager meets and works with individuals, interdisciplinary teams, and families/guardians to gather information; assess each individual's needs; assess service needs; prepares a plan of care; and identifies and implements services to meet an individual's needs. No Weekends! **ESSENTIAL JOB FUNCTIONS** _To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:_ + Provides case management services including intake, assessment, crisis intervention, advocacy, referral, and ongoing monitoring. + Completes QIDP responsibilities (if required). + Gathers social history to assess strengths and weaknesses, and to assess existing risks. + Conducts individual home visits and office base services, and accompanies individuals served as needed. + Coordinates care with Program Directors, Supervisors, DSPs, medical professionals, nutritionists and other service providers to prevent service duplication and to ensure the individual receives high quality care. + Identifies and procures group services appropriate for the individual to maximize their ability to live well in their home and community. Plan and assist in obtaining services from third-party service providers as needed. + Develops a plan of care for each individual to include services such as advocacy/case management, independent living skills, behavioral support, training, peer support, physical therapy, cognitive therapy, etc. + Monitors the delivery of services in coordination with the individual, company service providers and third-party service providers. + Assists individuals with related matters including their eligibility for benefits through Medicaid, Medicare, Social Security and private insurance. Acts as Representative Payee Designee, as required. + Maintains up-to-date case records with the status of case activity, including progress notes on all contacts. + Performs other duties as required. **SUPERVISORY RESPONSIBILITIES** None required. **Minimum Knowledge and Skills required by the Job** _The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:_ **_Education and Experience:_** + Bachelor's degree in human services or equivalent in education and experience required. + One year of related work experience and knowledge of case management. **_Certificates, Licenses, and Registrations:_** + Valid driver's license and driving record that meets insurability requirements as defined by the company, if applicable. Current CPR/First Aid Certification as required by state/program. + QIDP qualified (if required) **_Other Skills and Abilities:_** + Knowledge of case management + Ability to establish working relationships with individuals served + Demonstrated competence in verbal and written communication skills + Ability to handle crisis situations with minimal supervision + **_Language Skills:_** + Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, individuals served and the general public. + **_Mathematical Skills:_** + Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. + **_Reasoning Ability:_** + Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
